Ascended astarion
In Act 3 of Baldur's Gate 3ascended astarion, several character arcs come to a head—one of which is that of Astarion, the vampire spawn enslaved by the Vampire Lord Cazador for two centuries.
Shadowheart will embrace Shar and become a Dark Justiciar or return to her origins and serve Selune, for instance, and Baldur's Gate 3 's Gale wants control of the Crown of Karsus so he can surpass Mystra's power, or he will sacrifice himself to stop the Absolute. All companions have their own endings, and they usually range from good, bad, or tragic. That said, there is one companion whose endings are both tragic. If his personal quest is completed, he will either become a Vampire Ascendant or remain a Vampire Spawn. Both endings are tragic in their own way. However, there is one ending that is more tragic than the other, and that would be Astarion becoming the Vampire Ascendant.
Ascended astarion
Astarion is an Origin character and a companion in Baldur's Gate 3. He is portrayed by Neil Newbon and was created by Stephen Rooney. Astarion prowled the night as a vampire spawn for centuries, serving a sadistic master until he was snatched away. Now he can walk in the light and has the chance at a new life, but how long can he keep his past buried? Astarion starts as a Rogue. Upon reaching level three, he will default to the Arcane Trickster subclass. Astarion's companion quest is called The Pale Elf. His story unravels as Astarion explores his newfound freedom, coming to terms with the abuse inflicted on him, and that he has inflicted on others. Astarion can be found standing on a road west of the crashed Nautiloid in the Wilderness. When found, a scene will play where he asks for help defeating an Intellect Devourer. Afterwards, he can be asked to join the party. Astarion approves of displays of power and cruelty, and appreciates snarkiness and mocking. He is inspired when the player gains access to areas or information by deceiving others.

Either he becomes a Vampire Ascendant and gains incredible power, or ends the vicious cycle once and for all by not ascending. Ascending would sacrifice 7, vampire spawn. Not ascending would mean Astarion must rely on the tadpole to walk in the sun. If you decide to sacrifice the 7, vampire spawn so Astarion can ascend, a lot will change. If you want to live your vampire dreams, choose this option. If you choose to persuade Astarion not to go through with the ritual, you need to pass a pretty high skill check DC. After he calms down, you now have to decide what to do with the 7, vampire spawn. In my opinion, this is the much better ending for Astarion.
Ascended astarion

Upon entering the city, you'll be able to assist Astarion in confronting Cazador, who intends to execute a dark ritual by sacrificing seven-thousand spawn to amplify his own power. After subduing Cazador, Astarion will ask you to help him use the ritual for his own ascension.

Letting Astarion ascend makes him stronger and gives him new abilities, but the biggest differences between completing the ritual and stopping it are related to his story and personality. If Astarion ascends, his Bite ability is upgraded and he deals Necrotic Damage when he attacks.
