Aim of this study is to review our experience in the management of patients with synchronous and metachronous lesions, and reach conclusions regarding their optimal diagnosis, treatment and follow-up. Patients and methods: Between and , 12 patients seven men and five women, mean age The diagnosis lag for metachronous lesions ranged from 1. All three patients with synchronous cancers had two lesions. Results: Staging colonoscopy and abdominal CT was conducted in 10 patients while the remaining two underwent only abdominal CT due to their critical condition at presentation.

The highest occurrence rate of SPM, of Genitourinary malignancies were reported to be the third-most-common SPM in several reports. However, the incidence of genitourinary malignancy as the first primary cancer associated with SPM has not been reported. Several risk factors are related to the occurrence of SPM, including viral infection chemotherapy, radiation, genetics, smoking, betel quid chewing, and environmental factors. An early survey for SPM is indicated in first primary malignancy patients with these associated factors. In this study, we summarize several risk factors related to the occurrence of SPMs and preventive tests, which may help in their early detection and, consequently, better survival. Multiple primary malignancies are not uncommon in cancer patients, and they may involve risk factors such as genetics, viral infection, smoking, betel quid chewing, and environmental or treatment-related factors. The occurrence of multiple malignancies may be synchronous in the short-term or the long-term during follow-up. The definition of synchronous, multiple primary malignancies differs between studies. MPM was first described in , and then Warren and Gates extended the criteria to define synchronous tumors in [ 5 ]. These two tumors may be diagnosed at the same time synchronous , or the second tumor may be detected 6 months afterwards metachronous. A subsequent definition was provided by the Surveillance Epidemiology and End Results SEER project [ 6 ], which defined two or more primary malignancies diagnosed within a 2-month period as synchronous multiple tumors.

Breast cancer was the most frequent form of single primary cancer Hepatitis B and C virus are highly associated with hepatocellular carcinoma [ 23metachronous, 24 ]. The tumours were located more frequently metachronous the rectum, followed by the sigmoid colon Table 2.

Metrics details. The situation of patients developing multiple primary cancers is becoming more frequent and graver. This study investigated the risks of developing second primary cancers that are related to first primary cancers, and the interval times of synchronous and metachronous multiple primary cancers. A two-month period between first- and second- primary cancers was used to differentiate metachronous and synchronous multiple primary cancers. The combinations of subsequent cancers and relative risks RRs of having multiple primary cancers versus having single primary cancer for the top-ten first and second primary cancers were examined. The RR was adjusted for age of the first primary cancer.

Most After adjustment for age at first diagnosis, head and neck cancers had the highest metachronous association with second esophageal cancers RR,

It was also noted that most of the second primary cancers occurred more than 2 months after the first primary cancers had been diagnosed; the exception was female genital cancer—uterine, which tended to occur within 2 months. Late Sequelae of Radiotherapy. Two patients underwent a palliative resection because of liver metastases a patient with metachronous carcinoma , and locally extensive disease a patient with synchronous carcinomas, Table 3. Table 1 Clinical characteristics of the analytical cohort. Siriraj Cancer Registry. De Gonzalez A. Next were the median times following breast, skin, and colorectal cancer diagnoses each within 1 year. Conclusions The top-ten most frequent multiple primary cancers were the following: breast; liver; head and neck; colorectal; male genital cancer—prostate; skin; female genital cancer—uterine; thyroid; lung; and female genital cancer—non-uterine. From the 10 first multiple primary cancers of all cases, we predicted the RRs adjusted for age between the first and second primary cancers. In the remaining patient, the synchronous tumour was discovered intraoperatively emergency operation for bowel obstruction.

The preoperative diagnosis of multiple synchronous colorectal carcinomas remains difficult. Therefore, there might be a link between tumors such as sharing common risk factors or genetic linkage. The combination of first breast cancer and second breast cancer was statistically significant, compared with the combination of first breast cancer and other cancer types RR, Additionally, it contributes to cancer prevention allowing the discovery and removal of small polyps before their transformation. Carefully surveying primary cancer patients with many associated risk factors for SPM may facilitate the early detection of SPM. The top-ten most frequent multiple primary cancers were the following: breast; liver; head and neck; colorectal; male genital cancer—prostate; skin; female genital cancer—uterine; thyroid; lung; and female genital cancer—non-uterine. Germline BRCA mutations are associated with higher risk of nodal involvement, distant metastasis, and poor survival outcomes in prostate cancer. Table 2. Federal government websites often end in.
