Red Mutagens in particular are quite useful because they boost your attack power, but finding enough of them to use in your experiments can be a frustrating, time-consuming process. Although at the start of the game it will be a while, probably, before you see your first red mutagen. They are extremely rare to begin with, but seem to become more prevalent the higher level you become. The first red mutagen I got in my last playthrough appeared at level 15 and came from The Apiarian Phantom. During the early part of the game your best bet for getting hold of red mutagens are Drowned Dead on Skellige. There are two places where these monsters regularly re-spawn and are worth visiting to try to get red mutagens : the Whale Graveyard , and Redgill — both found on the east coast of the main island, Ard Skellig. What I do is: fast travel to the Whale Graveyard ; kill the Drowned Dead ; take their mutagens ; then jump on Roach and ride to Redgill.

So I tried to do the fiend farming trick, killed it once and got the lesser red, but meditating didn't seems to make it respawn. Is it already patched, or did I done something wrong? You can also get them from Arachas. Both should be south of Crookback Bog. Note that most say you need to be at least level 20 to get such loot though. The fiend and arachas one changed a few versions back so it doesn't work anymore. In Velen there is an alghoul that does respawn just southeast of Downwarren in a cluster of houses. For me I've been able to kill it and meditate and kill it again. After those 2 times i've had to travel between areas Velen, White Orchard, Skellige to get it to come back again. Otherwise from what i've read you'll start getting them more often in Skellige off the higher level stuff. Plus there is that Whale graveyard spot mentioned above. I have just gotten to Skellige in my current playthrough so I haven't tried it out yet. I haven't spent much time farming that alghoul. Its not a very high drop rate for red since you can also get others off of it too.

Farming red mutagens reliably is tricky in earlier levels of the game and stacking Geralt's attack power is rather minimal before you are able to craft greater red mutagens. That said, you will need a total of nine lesser red mutagens to get one greater and they drop rarely in the earlier parts of the game while you are still prowling Velen and Novigrad. Well, there is one reliable way to get them, thanks to a respawning Alghoul in a village called Toderas but there are a few prerequisites you need to complete before the necrophage can be farmed. The village itself is located directly east of Crow's Perch and directly south of Oxenfurt. You can pinpoint its location by the intersect of the lines from these two points.
