This character debuted in an ask blog created by a user named Ask-Dusttale on February 10th, He was a regular Sans who, upon learning of what the human has done, aims to stop their genocidal actions by slaughtering all of the Underground to acquire enough LV to defeat the human. Since his original debut, the character has gained high amounts of popularity, earning it thousands of pieces of fanart, comics, animations and even a full-fledged fangame.

Sans is the main enemy of the fan-game Dusttale: Last Genocide. His battle takes place in 3 Phases, with varying mechanics and different levels of difficulty. Sans frequently switches the Soul between Red to Blue Mode, which forces the Human to jump through gaps between his bones or slamming it against the walls of the Bullet Board. He often combines the latter with Gaster Blasters. As the final attack of Phase 1, he will use normal, light blue and orange bones alongside many Gaster Blasters, before concluding his onslaught with a circle of Blasters and trying to slam the Soul towards the walls where he'll fire more Blasters. In Phase 2, Sans will start to periodically slash the Bullet Board with his sword, generating a straight white beam. A flashing warning will appear in the place he'll strike. The screen will also be flipped right, left or upside down on certain attacks. Many of his attacks are used in conjunction with Papyrus. As the final attack, Sans will initially use a combination of both bones and blasters, before letting Papyrus take over.

Sans is the anti-heroic antagonist of the fan-game Dusttale: Last Genocide. This attempt never came to fruition, with him being defeated countless times. This came to an halt after Gaster restored his memories, prompting Sans to put an end to the cycle once and for all. Unlike his previous self, Sans appears much more determined and less lazy. He still keeps a joking mood around himself, although in a more grim and threatening manner. He deeply regrets of having committed his murderous actions, expressing regret but, after Gaster's intervention, he focuses on not making a single death be in vain saying how he "will bring [the Human] to justice]". His will to end Monsterkind suffering once and for all has pushed him to extreme actions, first by becoming a murderer and then by attempting to harness the power of a mysterious Experiment. After countless Genocide Runs committed by the Human , Sans somehow came to realise of the events that were about to transpire, which brought him to harness the power of LOVE, slaughtering Monsters, including his brother , in order to stop the Human. Despite this, the Human still proved victorious, killing Sans over and over again, resetting each time they won, forcing Sans to repeat the cycle by making him forget.
